Vanpool Application Packet Checklist
The following materials must all be included in a packet in order to start your vanpool group.
Complete and sign the forms. Please review the forms carefully – missing items may cause the
start date of the vanpool to be delayed.

___ Copy of Current Insurance Card (front and back)
___ Copy of Driver’s License (front and back)
___ First Month’s Fare
ALTERNATE DRIVER – Vanpool group must have at least one (1) alternate driver
